Position Summary:

Part time position (20 hrs/wk) - Provide behavioral health consultation, brief assessment, prevention and interventions to adults, adolescents, children and their families within their medical home to improve psychosocial functioning. Bilingual Spanish required.

Salary Range: LCSW $24.86/hr. - $29.81/hr.

Responsibilities include the following: 

  1. Evaluate crisis situations and apply appropriate interventions. 
  2. Actively participate in meetings that support the Primary Care Clinic(s) integrated health care model to provide comprehensive care for clients. 
  3. Assist in the detection of "at risk" patients and development of plans to prevent further psychological or physical deteriorations. 
  4. Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease. 
  5. Provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders. 
  6. Teach patients, families and staff care, prevention and treatment enhancement techniques. 
  7. Assist in the development of effective infrastructure to support the model of care developed. 
  8. Provide diagnostic mental health assessments as well as individual, couple and/or group psychotherapy and crisis interventions to assigned clients (both children and adults). 5. Address PPD through individual and/or group sessions. 
  9. Facilitate the coordination of medication/medicinal needs of clients with psychiatric and medical resources. 
  10. Support children, adolescents and their families using a variety of developmentally appropriate strategies (e.g. treating trauma across the lifespan). 
  11. Collaborate with internal and external partners in support of the Individual/family’s growth and development. 
  12. Maintain accurate and timely clinical documentation consistent with agency standards. 
  13. Participate in team meetings, in-services and supervisory sessions as required.
  14. Other duties as assigned. 

Job Requirements


Master’s Degree in psychology, social work, or other related human services degree, Licensed as an LCSW, LPC or LMFT by the State of Colorado


Three to five years experience working with Adults, Adolescents, Children and Families. Excellent knowledge of behavioral medicine and evidence based treatment for medical and mental health conditions. Experience collaborating with a Multidisciplinary team. Familiarity with a primary care/pediatric environment is a plus. Bilingual Spanish Required.


Cross-systems relationship development skills. Knowledge of methods of assessment and psychotherapy. Ability to assess crisis situations and intervene appropriately. Familiarity with the DSM V and diagnostic techniques. The ability to focus on what people can do, not on what they can’t do. The ability to partner with clients and families and work alongside them as they shape their wellbeing and progress. The ability to collaborate and work effectively on a multidisciplinary team. The ability to laugh with others and have a sense of humor. Effective written and verbal communication skills. Ability to work flexible hours Knowledge of methods of assessment and psychotherapy. To perform this job successfully, an individual should have knowledge of Word Processing software and familiarity with Electronic Health Records. 


Ability to sit, stand, bend and reach, drive, operate standard office equipment such as phone, computer, facsimile, copier/printer, and other business machines. Occasional lifting up to 40 lbs.


Ability to give, receive and analyze information, formulate work plans, prepare written materials and articulate goals and action plans. Must understand people from all walks of life and be able to communicate effectively with them.
