Job Description

POSITION SUMMARY:  Bilingual Spanish/English speaking Resource Coordinator/Clinical Case Manager needed for two locations - Dahlia Campus for Health and Wellbeing and A New Day Family Resource Center.

Provide resource coordination and daily operations onsite support for A New Day Family Resource Center and the People’s Food Pantry.  Administrative duties as applicable to daily operations of the People’s Food Pantry and the Healthy Living Team. Facilitation of Spanish speaking groups as needed annually for Incredible Years grant requirements.

SALARY RANGE:

CMI $18.90 - 22.70/hr.

CMII $21.13 - 25.37/hr.

ESSENTIAL JOB FUNCTIONS:

  • Create a positive experience for community members and people receiving services through professional conduct and customer service through the sign up/in station. Ensure intake spaces are aesthetically pleasing, safe, and friendly for all guests.
  • Receive and screen pantry telephone communications ensuring callers are directed to the appropriate destination in a timely fashion.
  • Assist applicants in intake/orientation processes.
  • Maintain each consumer’s HIPAA rights, and other protections of confidentiality and privacy.
  • Read, understand, and excel in procedures for maintaining consistent standards of the paper and electronic medical record (EMR), which is outlined in the HISM manual and in client database system.
  • Maintain Outlook calendar, tasks, and meetings and fulfill email requests as appropriate.
  • Ensure prompt, accurate, and efficient handling of documents for filing and scanning.
  • Compose some routine correspondence.
  • Generate and submit necessary reports to Program Manager.
  • Maintain submission of surveys online and collect data when requested by Program Manager.
  • Translate documents for Healthy Living programs as needed.
  • Copy/scan/fax information as required.
  • Complete requisitions for needed office supplies and equipment.
  • Perform a variety of administrative duties to ensure smooth running of the pantry.
  • Follow organizational and departmental rules and guidelines.
  • Perform other duties as assigned, based on individual site needs.
  • Facilitation of Healthy Living Spanish speaking groups as needed.
  • Make full utilization of the consumer’s support systems in order to encourage treatment program engagement.
  • Reach out to program eligible consumers, inform them of and educate them about available services at MHCD and in the community.
  • Other duties as assigned.

Job Requirements

EDUCATION:

Bachelor (CM I) or Master (CM II) Degree in psychology, social work, or other related human services degree. 

Must have valid Colorado Driver license and a recent clean driving record. Ability to transport consumers in dependable vehicle.

As a Clinical Case Manager you will be providing transportation to the people we serve in your vehicle. You are required to carry Business Use coverage on your policy (additional cost is reimbursed).

EXPERIENCE:

Minimum one-year experience in mental health or human services.             

SKILLS AND COMPETENCIES:

  • Knowledge of methods of psychotherapy.
  • Knowledge of community resources.
  • Ability to assess crisis situations and intervene appropriately.
  • Familiarity with the DSM V and diagnostic techniques. Understanding of consumer’s right to privacy and confidentiality (basic HIPAA principles).
  • Effective written and verbal communication skills. Ability to work flexible hours which may be required.
  • Ability to give, receive and analyze information, formulate work plans, maintain confidentiality, prepare written materials and articulate goals and action plans.
  • Must understand people from all walks of life and be able to communicate effectively with them.
  • Must have a clear understanding of HIPAA, confidentiality, personal boundaries and be self-assured in a variety of situations.
  • Have the ability to de-escalate and remain calm.
  • Someone who is energetic, with a positive and creative attitude is necessary.
  • Knowledge and understanding of trauma-informed care principles/practices.  

SUPERVISORY RESPONSIBILITIES:  

None

PHYSICAL REQIUREMENTS:  

Ability to sit, stand, bend and reach, drive a vehicle, operate standard office equipment such as phone, computer, facsimile, copier/printer, and other business machines. Occasional lifting up to 40 lbs. Must have safe, dependable vehicle to transport consumers with a valid Colorado Driver License/good driving record.

WORKING CONDITIONS:

May require occasional evening, or weekend hours depending on program and consumer needs.
