Job Description

POSITION SUMMARY: Provides comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each mental health consumer’s abilities to function in the community. Assesses, monitors, and follows up with consumer’s ability to meet basic needs in moving toward recovery.  Responsible for all clinical documentation of consumer interaction.

SALARY Range:  
CMI –  $17.58/hr. – $21.12/hr. (Bachelor degree)

  • Coordination of care/services with MHCD as well as outside providers including linking consumers to out of state services and additional support systems- Primary Care Physician (PCP), Long Term Care, RTD, DHS, etc. and assisting in paperwork to these additional support systems.
  • Outreach – Keeping consumer’s engaged in services that have not come in. This is done by phone, letters, nurses, front desk, 2 Succeed, etc.
  • Offer extra support to consumer. We are the contact person for consumer. This includes help with problem solving and community support by phone.
  • Documentation – Keeping consumer’s charts updated such as RNL’s every 6 months, CCAR’s, Recovery Markers and Measures. Goal-mastery (attend groups only) consumers include additional documentation of 6 month treatment plans and case reviews.
  • Complete ROI’s, current demographic, income, insurance updates, emergency contact, Primary Care Physician (PCP).
  • Assess consumer’s need for higher levels of care or readiness to move to next level of recovery with PCP in the community.
  • Assist prescribers as requested with consumers not on caseload for case management services, filling out forms for prescribers, drafting letters to be signed by prescriber that was requested by consumer.
  • Facilitate groups
  • Case management (housing needs)
  • Submitting referrals for consumers internally or externally
  • Assist in obtaining insurance or prescription assistance
  • Advocate for consumers
  • Discharge summaries and transfer assessments

EDUCATION: Bachelor (CM I) Degree in psychology, social work, or other related human services degree.

EXPERIENCE: Minimum one-year experience in mental health or human services.


  • Knowledge of methods of psychotherapy.
  • Knowledge of community resources.
  • Knowledge of Mental Health Center of Denver services and resources in the community
  • Ability to assess crisis situations and intervene appropriately.
  • Familiarity with the DSM V and diagnostic techniques. Understanding of consumer’s right to privacy and confidentiality (basic HIPAA principles).
  • Effective written and verbal communication skills. Ability to work flexible hours which may be required.
  • Ability to give, receive and analyze information, formulate work plans, maintain confidentiality, prepare written materials and articulate goals and action plans.
  • Must understand people from all walks of life and be able to communicate effectively with them.
  • Must have a clear understanding of HIPAA, confidentiality, personal boundaries and be self-assured in a variety of situations.
  • Have the ability to de-escalate and remain calm.
  • Someone who is energetic, with a positive and creative attitude is necessary.
  • Knowledge and understanding of trauma-informed care principles/practices.


PHYSICAL REQIUREMENTS:   Ability to sit, stand, bend and reach, operate standard office equipment such as phone, computer, facsimile, copier/printer, and other business machines.  Occasional lifting up to 40 lbs.

WORKING CONDITIONS:  May require occasional evening hours.
