POSITION SUMMARY: Provides connections to comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each person receiving mental health services the ability to function in the community. Focus on providing linkage to services for individuals in county court who have a history of multiple competency filings.

STARTING SALARY:

CMI - $18.90/hr. - $22.70/hr. (Bachelor’s Degree)

 ESSENTIAL JOB FUNCTIONS:

  1. Conduct face to face behavioral health assessments, diagnosis and evaluation in the Denver Downtown Detention Center flagged for this pilot diversion program.
  2. Responsible for all clinical documentation of consumer interaction.
  3. Provide assistance for those receiving services and involved in the criminal justice system where competency has been raised in meeting basic needs for food, clothing, shelter, personal safety, and general medical and dental care.
  4. Assist with applications for income, medical, housing, or other benefits which they may need and to which they are entitled.
  5. Identify and establish if a protective payee-ship is necessary and arrange this assistance. 
  6. Ensure access to needed services and community resources by arranging for transportation, or if necessary, providing transportation to the service in your personal vehicle (business insurance on vehicle reimbursed by MHCD).
  7. Provide comprehensive psychosocial services that include a continuum of high and low intensity services designed to improve or maintain abilities to function in normal social roles.

These will include, but be limited to, services which:

  • Evaluate strengths and symptoms. Facilitate setting their own goals, and plan for appropriate services;
  • Guide and instruct in daily living skills such as medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, use of transportation, and other community services.
  • Help develop social skills, interests, and leisure time activities to provide a sense of participation and personal worth, including opportunities for age appropriate, culturally appropriate daytime and evening activities.
  • Assist with employment opportunities, vocational rehabilitation services, or supported work environments where appropriate.
  • Assist with special living arrangements in locating a rehabilitative or supportive housing arrangement. The choices should be broad enough to allow each person receiving services the opportunity to live in an atmosphere offering the degree of support necessary.
  • Offer support, assistance, consultation, and education to families, friends, landlords, employers, and the community. Maximize benefits and minimize problems associated with the presence of these persons in the community.
  • Assist with navigating the criminal justice system and moving their court case through the system efficiently.
  • Collaborate with system stakeholders as appropriate.
  • Collaborate with other forensic branches of the Crisis Intervention Response Unit to ensure appropriate wrap around services and service linkage.
  1. Make full utilization of the consumer’s support systems in order to encourage treatment program engagement.
  2. Reach out to people who are eligible for program to inform and educate them about available services at MHCD and in the community.
  3. Other duties as assigned.

Job Requirements

REQUIREMENTS:

EDUCATION:

Bachelor (CM I)Degree in psychology, social work, or other related human services degree.

Must have valid Colorado Driver license and a recent clean driving record. Ability to transport consumers in dependable vehicle.

 As a Clinical Case Manager, you will be providing transportation the people we serve in your vehicle. You are required to carry Business Use coverage on your policy (additional cost is reimbursed).

 EXPERIENCE: One-year experience in mental health or human services preferred.

SKILLS AND COMPETENCIES:

  • Knowledge of methods of psychotherapy.
  • Knowledge of community resources.
  • Ability to assess crisis situations and intervene appropriately.
  • Familiarity with the DSM V and diagnostic techniques. Understanding of consumer’s right to privacy and confidentiality (basic HIPAA principles).
  • Effective written and verbal communication skills. Ability to work flexible hours which may be required.
  • Ability to give, receive and analyze information, formulate work plans, maintain confidentiality, prepare written materials and articulate goals and action plans.
  • Must understand people from all walks of life and be able to communicate effectively with them.
  • Must have a clear understanding of HIPAA, confidentiality, personal boundaries and be self-assured in a variety of situations.
  • Have the ability to de-escalate and remain calm.
  • Someone who is energetic, with a positive and creative attitude is necessary.
  • Knowledge and understanding of trauma-informed care principles/practices.  

 SUPERVISORY RESPONSIBILITIES:  

None

PHYSICAL REQIUREMENTS:  

Ability to sit, stand, bend and reach, drive a vehicle, operate standard office equipment such as phone, computer, facsimile, copier/printer, and other business machines. Occasional lifting up to 40 lbs. Must have safe, dependable vehicle to transport consumers with a valid Colorado Driver License/good driving record.

WORKING CONDITIONS:

May require occasional evening, or weekend hours depending on program and consumer needs.
