POSITION SUMMARY: Provides comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each mental health consumer’s abilities to function in the community. Assesses, monitors, and follows up with consumer’s ability to meet basic needs in moving toward recovery. Responsible for all clinical documentation of consumer interaction. As a Clinical Case Manager you will be providing transportation the people we serve in your vehicle (additional cost of insurance reimbursed).


CMI - $18.90/hr. - $22.70/hr. (Bachelor’s degree)

CMII - $21.13/hr. - $25.37/hr. (Master’s Degree)


QMAP (Qualified Medication Administration Person)

CPR & First Aid

Physical and TB required. (must be completed prior to hire, and maintained during employment)


  • Help consumers in meeting basic needs for food, clothing, shelter, personal safety, and general medical and dental care, and assist them with applications for income, medical, housing, or other benefits which they may need and to which they are entitled.       Identify consumers for whom establishment of a protective payee-ship is necessary and arrange this assistance.
  • Ensure consumers access to needed services and community resources by arranging for transportation, or if necessary, by taking the consumers to the service in personal vehicle (business insurance on vehicle reimbursed by MHCD).
  • Provide comprehensive psychosocial services that include and continuum of high and low intensity services designed to improve or maintain consumers’ abilities to function in normal social roles. These will include, but be limited to, services which:
    • Help consumers evaluate both their strengths and symptoms, and facilitate in consumers setting their own goals, and plan for appropriate services;
    • Guide and instruct consumers in daily living skills such as medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, use of transportation, and other community services;
    • Help consumers develop social skills, interests, and leisure time activities to provide a sense of participation and personal worth, including opportunities for age appropriate, culturally appropriate daytime and evening activities;
    • Help consumers find and make use of appropriate employment opportunities, vocational rehabilitation services, or supported work environments where appropriate;
    • Assist consumers who need special living arrangements in locating a rehabilitative or supportive housing arrangement. The choices should be broad enough to allow each consumer and opportunity to live in an atmosphere offering the degree of support necessary, while also providing incentives and encouragement for consumers to assume increasing responsibilities for their lives;
    • Offer support, assistance, consultation, and education to families, friends, landlords, employers, community agencies, and others who come into contact with consumers, in order to maximize benefits and minimize problems associated with the presence of these persons in the community;
  • Identify and work with potential natural support systems such as neighborhood networks, churches, and community organizations.
  • Make full utilization of the consumer’s support systems in order to encourage treatment program engagement.
  • Reach out to program eligible consumers, inform them of and educate them about available services at MHCD and in the community.

Must have valid Colorado Drivers license and a recent clean driving record. Ability to transport consumers in dependable vehicle. As a Clinical Case Manager you will be providing transportation the people we serve in your vehicle. You are required to carry Business Use coverage on your policy (additional cost reimbursed).

Job Requirements

EDUCATION: CM I - Bachelor's Degree in psychology, social work, or other related human services degree.

CM II - Masters Degree in psychology, social work, or other related human services degree.

One year experience in mental health or human services preferred.

Specific duties:

Complete all Intakes:

-Beds need to be filled within 48 hours of vacancy. Contact with PM should be made if the bed is open more than 2 business days.

-Use Residential Substance Abuse Referral waitlist

-Contact the case manager of the next person on the list to ensure consumer is not in jail, interested in Beeler, and not in detox

-Anyone at Beeler in the last 6 months needs PM approval

-All consumers must have CBI prior to being considered

-Check Avatar file for assaultive or violent hx that has not been reported, ensure they are not a sex offender

-PM must approve all admits of consumer with assaultive or violent histories 

Before Admission ensure we have:

-Pre admission screen signed by doctor in Avatar. If they do not have a doc route to PM for signature

-Face Sheet

-Sign medical and Psychiatric orders

-Signed PRN order

-Signed parole or probation conditions if they are on parole or probation

-Current TB within the last year 

At Admission

-Schedule first staffing according to Beeler schedule.

-work with PM to assign intern for individual therapy and set up first session if possible

-Orient consumer to the house

-Complete intake packet

-Complete Board and Care plan within 24 hours- route to doc and PM

-Complete Dacod and Socrates

-Provide consumer with linens

-Cook or supervise washing of belongings 


-DC planning in staffings to include: housing, medical care, psychiatric care, social supports, relapse prevention plan, and recreational plan

-Complete discharge summary in Avatar and provide documentation to PO and the team as needed

-Break down charts and sent to scanning 


-Facilitate groups as assigned according to group schedule

-Facilitate morning goals group according to group schedule 


-Transport residents in personal vehicle as needed

-Send Beeler House and PM list of the following week’s intakes by 4pm Thursday

-Schedule staffings during assigned periods. Staffings need to occur at least 3 times during a 90 day stay. Inform Beeler house and PM of scheduled staffings the prior Thursday by 4pm

-Track admission and discharge dates

-Collect rents and keep record monthly in a spread sheet sent to Administrative Assistant (currently Jillian McNaughton)

-Schedule and attend all staffings with RC, PM, and parole or probation if possible.

-Send staffing notes to the team within 24 hours of meeting. Keep staffing notes in a staffing binder

-Do work in the milieu

-Discuss individual therapeutic interventions with program manager in supervision prior to performing

-Other duties as assigned.


MACHINES AND EQUIPMENT TO BE USED: Computers, calculators, fax machines, copiers, telephone and a variety of other office/clerical equipment. 

TYPICAL PHYSICAL DEMANDS: Requires sitting, standing, bending and reaching. May require lifting up to 50 pounds.   Requires manual dexterity sufficient to operate standard office machines such as computers, fax machines, the telephone and other office and/or clinical equipment. 

WORKING CONDITIONS: May require occasional evening hours.
